Home Based Services

Shore offers Home-Based Services (HBS) to individuals, ages three to adult, with an intellectual or other developmental disability who have received an Illinois Department of Human Services award letter that states they have access to funding for services provided by the HBS program.

As of January 1, 2024, the monthly allotment for qualified HBS participants is:

  • $2,829 for adults
  • $1,886 for children

 

What are Home-Based Services?

The purpose of HBS is to help individuals to continue living safely and successfully in their own home or with their family, with the overall goal of promoting maximum growth and independence for the individual. HBS is a self-directed program in that individuals, guardians and families have the responsibility to arrange and manage their own services and supports, which ultimately serve the needs and outcomes of the individual’s Personal Plan.

Services available for purchase include:

  • Self-Direction Assistance (SDA)
  • Personal Support Work (PSW)
  • Community Day Services
  • Developmental Training
  • Employment Services

 

Self-Direction Assistance (SDA) is an optional waiver service for purchase that is available in both the Children’s and Adult Home-Based Waivers. A Self-Direction Assistant can help with arranging for, directing and managing PSW services. They can also provide skills training designed to better recruit and hire PSWs and provide information on effective communication and problem-solving skills.

Eligibility Requirements

Intake Process

  • The Independent Service Coordination (ISC) agency contacts Shore and sends a referral packet.
  • The family then contacts Shore.
  • The Self-Direction Assistant from Shore conducts a home visit and completes required documents with the family.
