Community Day Services
The program serves individuals 18 years and older with:
- Multiple disabilities, including severe/profound to
moderate disabilities - Physically challenged
- Medically fragile
The program is designed to provide training in:
- Adaptive daily living skills
- Prevocational job skills
Activities include:
- Community integrated activities
- Healthy living & exercise
- Art & leisure activities
- Healthy living & exercise
Staff Include:
- Professionals and paraprofessional
- Full-time nurse
- Consulting services may be available: speech therapy, physical therapy, occupational therapy
- Services are provided to stimulate the motor, language, cognitive and social development of the adults in order to promote their functional independence and ensure the fullest integration possible into their community living environment.

Eligibility Requirements
Individuals with intellectual or other developmental disabilities who need more assistance in their daily activities.
How to Apply
- Initial tour of program
- Completion of Intake Packet, which includes: admission information (e.g. Psychological, Physical, Education, Program Plan, etc.)
- Participants should be recipients of Medicaid funding otherwise private pay is an option.
