Buehler House/Shore Homes East is a licensed 12 bed Intermediate Care Facility for the Developmentally Disabled (ICF-DD) located in Evanston which provides 24-hour staff supervision. The goal is to provide an environment as similar as possible to everyday life in mainstream society. Residents are encouraged to interact within the community and to become less dependent on others. Moving residents to more independent living environments is a main focus.
Community Living Facility
Shore Homes West is a licensed 12 bed Community Living Facility (CLF) located in Skokie which provides 24-hour staff supervision. The goal is to provide an environment as similar as possible to everyday life in mainstream society. Residents are encouraged to interact within the community and to become less dependent on others. Moving residents to more independent living environments is a main focus.

Eligibility Requirements
- Individuals with an intellectual and/or other developmental disability.
- 18 years of age and older, functioning in the mild to severe range and not dangerous to themselves or others.
- Sufficient medical stability to live in a group setting without frequent on-site medical care, and capable of self-preservation in an emergency.
- Recent application filed for social security benefits or receipt of such benefits.
- Employed in a competitive or vocational work setting or enrollment in an external day program.

Intake Process
- Initial tour of residential home
- Completion of Intake Packet
- Intake Interview
- Submission of admission information; i.e., psychological, medical reports, social history
- Intake Committee review
